Tell Us What You Think about 'Transition' Services - Rainbow Resource

A local information service for families with a child or young person with additional needs, a special educational need, or disability is asking for young persons and carer feedback on 'transition' (the change from children to adult services).

Rainbow Resource is calling for the views and experiences of both young persons and their parents/carers to help the Local Authority improve the transitions process for young people. 

Transition’ is when a young person turns 18 years old, and the services accessed change from children’s services to adult services.

If a young person had a children’s services Social Worker or received support from another professional such as Child & Adolescent Mental Health Services (CAMHS), they will instead receive support from adult services when they reach 18.

Rainbow Resource has created a short survey for both young persons and parent/carers to complete to inform and improve the service.
