Learning Disability Hospital Passport and Guidance

The Grab and Go form gives doctor and nurses important information about a patient who has a learning disability or autism if they have to be admitted to hospital because of COVID 19. Find out more.
Inside a hospital room

The Grab and Go guide has been designed in partnership with people with learning disabilities, families and nurses.

It gives the information that doctors and nurses will need if you go to hospital because of COVID-19 and, for example, are struggling to breathe.

It is not a replacement for the everyday, detailed hospital passport. You should update your hospital passport and take that to hospital along with the Grab and Go guide if you need to be admitted.

How do I get a fit note?

From 1 July 2022, nurses, occupational therapists, pharmacists and physiotherapists can legally certify and issue a statement of fitness for work, or informally known as a ‘fit note’. Find out what a ‘fit note’ is, when you might need one and how to go about getting one. Previously only GPs or hospital doctors could issue fit notes.
